Abstract

Purpose: As anesthetic methods for dental implant surgery, local anesthesia, conscious sedation and general anesthesia with endotrachial intubation were selected. The purpose of this comparative study was to suggest a guide to the clinicians for the selection of methods of anesthesia in the dental implant surgery.

Methods: The patients who had been operated in the Seoul National University Bundang Hospital, from Jun 2003 to May 2004 were included. To evaluate factors associated with osseointegration, patients were classified according to gender, age, type of systemic disease, type of anesthesia. Also, subjective levels of pre-operative anxiety, intra-operative anxiety and intra-operative pain were investigated using visual analogue scale (VAS) method.

Results: There were no significant differences among groups at the level of pre-operative anxiety (p=0.759). However there were significant differences between monitored anesthesia care (MAC) group and local anesthesia (L/A) group at the level of intra-operative anxiety (p=0.030). Also, intra-operative pain level of MAC group was lower than that of L/A group (p=0.046). There were significant changes of anxiety level between pre- and intra-operative anxiety in the L/A group (p=0.001) and the MAC group (p=0.002).

Conclusion: From the analysis of preliminary data, most of the dental implant surgery can be performed under the L/A, but it is unsuitable for the timid or medically compromised patients or in the extensive surgical cases. Intravenous conscious sedation superiorly reduced the level of intra-operative anxiety and intra-operative pain to local anesthesia. More safe treatment can be supplied to the timid or medically compromised patients under the conscious sedation with close monitoring.
